What we do

Skilled nursing at home, and the paperwork off your plate.

In-home skilled nursing

RNs and LPNs deliver private-duty nursing in the family's home for children under 21 with complex medical needs, trach and vent, G-tube, oxygen, seizure management, and more.

We own the paperwork

GAPP prior approval, the Plan of Treatment, medical necessity, and Medicaid coordination. We handle the process end to end so your team and the family aren't buried in forms.

We coordinate with your team

We work alongside discharge planners, case managers, social workers, and physicians, and keep you updated on where each referred patient stands.

Continuity as they grow

We support medically complex GAPP children over time, including the transition toward COMP and NOW waivers as they age out of the pediatric program.

The problem we solve

The gap between the hospital and home.

Medically complex children are ready to leave, but the path to safe home care is tangled in coverage, paperwork, and follow-through. That is exactly the gap HeartPath exists to close.

Discharges stall waiting on home nursing.
We can begin planning while the child is still admitted, so skilled nursing is ready when the family gets home, freeing the bed sooner.
Families get lost in the GAPP and Medicaid maze.
We guide the family through eligibility and applications, and complete the GAPP documentation for them, start to finish.
Families are told they earn too much for Medicaid, and give up.
We screen for the Katie Beckett pathway, which looks only at the child's needs and ignores parents' income, a common door into Medicaid and then GAPP.
Parents are left providing skilled care alone.
We place professional RN and LPN hours in the home so recovery happens safely, and caregivers can rest and go back to work.
Referrals stall with no follow-through.
We respond quickly, take ownership of the case, and close the loop with your team so nothing falls through the cracks.
Who to refer

Children with ongoing skilled nursing needs.

  • Tracheostomy and ventilator dependence
  • Feeding tube (G-tube / GJ) support
  • Oxygen and respiratory monitoring
  • Seizure disorders and rescue medication
  • Complex or high-risk medication regimens
  • Technology-dependent, medically fragile children

Not sure a patient qualifies? Refer anyway. We determine GAPP and Medicaid eligibility and tell you what is possible, at no cost to the family.

How it works

A referral takes a minute. We take it from there.

  1. 1
    Send us the patient
Call, email, or send a secure referral. A name and a way to reach the family is enough to start.
  2. 2
    We verify and file
We confirm Medicaid or start Katie Beckett, then complete the GAPP prior approval and Plan of Treatment.
  3. 3
    Care starts, you stay informed
We staff the case, begin nursing in the home, and keep your team updated on the outcome.

We aim to respond to referrals quickly, during business hours, Monday through Friday.
