Your name, phone number, email, and the details you submit through our forms and the Path Finder (for example, your child’s general care situation and your eligibility questions) so we can respond and help you understand your options.

IP address, device and browser information, pages visited, and referring links, collected through cookies and similar technologies described below.

3. Tracking technologies we use

• Google Analytics (GA4) — website usage analytics, to understand which pages and outreach are useful. Opt out with the Google Analytics Opt-out Browser Add-on.
• Meta (Facebook) Pixel — advertising measurement and retargeting. Manage your ad preferences at facebook.com/adpreferences.

We do not use advanced matching that would attach your medical or eligibility answers to advertising data. Path Finder answers are never sent to advertising platforms.

To respond to your inquiry, help you understand GAPP and related programs, provide and coordinate our services, improve this site, and measure the effectiveness of our outreach.

5. Sharing

We share information only with the service providers named above (analytics and advertising measurement) and vendors that help us operate the site and communicate with you. We do not sell your personal information.

We keep form and inquiry submissions only as long as needed to serve you and meet our record-keeping obligations, then delete or de-identify them.

Information you submit here is for initial outreach and eligibility guidance and is not a substitute for clinical records. Care and clinical documentation, once you become a client, are handled under our HIPAA obligations separately from this website.

9. Children

Our services support families of medically fragile children, but this website is intended for parents and guardians. We do not knowingly collect personal information directly from children under 13.

10. Georgia & U.S. residents

Depending on your state, you may have rights to access, correct, or delete your personal information, or to opt out of certain uses. Contact us at admin@heartpathcare.com to exercise them.
