From Your First Call to a Nurse in Your Home

Here's exactly what the Georgia Pediatric Program (GAPP) process looks like — and where HeartPath does the heavy lifting so your family doesn't have to.

The GAPP path

The step-by-step path

Here's the road from your first call to a nurse in your home — and where HeartPath carries the paperwork for you.

  1. Day 1
You reach out — free eligibility review

A coordinator calls to understand your child's needs and confirm whether GAPP, Katie Beckett, or another pathway fits. No cost, no obligation.

  2. Week 1
We gather documents & confirm Medicaid

We help line up your child's Medicaid, records, and the pieces the state needs — and start the Katie Beckett pathway if income was the barrier.

  3. Weeks 1–2
    Physician plan & packet prepared

We coordinate the physician plan of treatment and prepare the full GAPP packet on the official state forms — you just sign.

  4. Weeks 2–3
Submitted to Georgia Medicaid / GAPP

HeartPath submits the complete packet for prior authorization. We track it and handle anything the state comes back with.

  5. ~30 days
    State review & authorization

Georgia Medicaid reviews the packet and authorizes hours based on your child's documented needs. Reviews typically take about 30 days.

  6. After approval
Approved — care begins at home

We match your child with a skilled pediatric nurse experienced in their specific needs, and care starts at home.

Timeframes are typical ranges, not guarantees — eligibility and hours are determined by Georgia Medicaid based on your child's documented medical needs.
